Staff Only Flu Shot Clinic and Consent Forms
The Staff Flu Shot Clinic will be held on Thursday, October 16th, from 2:45 to 4:15 pm. The VNA Nurse will be here at 12:30 for student flu shots. If a teacher is only in the building part of the day, they may come anytime after 12:30.
Please print and complete the appropriate consent form, then bring it and a copy of your insurance card to the Principal's conference room on the day of the flu shot clinic.
